Parent consent

If you have been invited to the UPRO private beta, this is where you give consent for your child to take part. The consent form is hosted on Google Forms and is operated by U-Pro Soccer Technologies Inc.

You will be asked who you are, where you live, your child’s date of birth, and which permissions you want to give. Three of the five permissions are optional and are off by default. You can change or withdraw them at any time by emailing legal@uprosoccer.ca.

After you submit the form, you book a short welcome video call with us. That call is how we confirm you are the child’s parent or guardian. Your child’s access to the beta starts after the call.
